eMedic Invoice Form

This form displays a customized invoice with a price quote. The information is not sent to eMedic and nothing will ship until payment is received. This simply displays an invoice on your screen that you can print. It also lets you view volume pricing of various quantities.
All orders must be prepaid--Iterum does not accept purchase orders. You can print and submit this invoice to your purchasing department. Be sure to return the invoice with your check.
Additional discounts are displayed if you order five or more product licenses. The discounts increase based on the number of licenses purchased.
A mail order handling fee of $4.95 is added if you order only one license for a handheld computer by mail.
